As a doctor how to prescripe MINIRIN (desmopressin acetate) nasal spray for treatment of nocturnal enuresis ?

What is the initial dose to begin with ,what is the mentainance dose ,how long should it be administered and how to stop the treatment ,would it be be sudden or gradual withdrawal ,and if gradual ,how?
